To request an appointment

  1. Download the Evergreen Life app and link to the practice using NHS Login (If you do not have an NHS login, contact the practice and ask for your NHS Linkage Key instead).
  2. After you are linked, tap on the GP section on the home page and then tap ‘askmyGP’.
  3. It may ask if you already have an askmyGP account. If you have not used askmyGP before select ‘No’.
  4. You can now tap ‘Consult a Clinician’ and follow the instructions to inform the GP of the issue.

If you cannot use the app, please use the askmyGP service on the practice website. We will respond within normal working hours.

When you get in touch, we’ll ask what you need help with.

We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

Your appointment

However you choose to contact us we may offer you a consultation:

  • by phone
  • face to face at the surgery
  • on a video call
  • by text or email.

Appointments by phone, video call or by text or email can be more flexible and often means the doctor or nurse can help you sooner.

Emergencies

These will be dealt with as soon as possible at all times.

In life-threatening emergencies such as severe bleeding, collapse, difficulty in breathing, unconsciousness, stroke and severe chest pains, telephone 999 immediately.

Please tell us:

  • If there’s a specific doctor, nurse or other health professional you would prefer to respond.
  • If you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email.
  • If you need an interpreter.
  • If you have any other access or communication needs.

Home visits are made at the discretion of your doctor.

If you need to see the doctor but are not well enough to get to the surgery please telephone on 0208 517 1175 before 10.00am.

Please remember that a doctor can see six patients in the surgery in the same time as one house-call. On occasions the doctor will need equipment only available in the surgery. Please can you make every effort to get to the surgery using help from a relative, neighbour or taxi if necessary.

House calls are only available for patients who are medically unable to come to the surgery. It is for the doctor to decide whether it is appropriate to visit.

If you request a house call, our receptionist are under instruction to obtain sufficient information so that the doctors can judge whether a home visit is necessary and how urgent it is. Please anticipate appropriate questioning.

GP visit recommended

GP home visits makes clinical sense and is the best way of giving a medical opinion in cases involving:

  • the terminally ill
  • the truly bed-bound patient, for whom travel to premises by car would cause deterioration in their medical condition or unacceptable discomfort.

GP visit not recommended

In most cases with some exceptions mentioned above, a home visit would not be considered appropriate use of a GP’s time:

  • common symptoms of childhood: fevers, cold, cough earache, headache, diarrhoea /vomiting and most cases of abdominal pain. These patients are usually well enough to travel by car. It is not harmful to take a child with a fever outside. In a case when children may not be fit to travel by public transport or walk the patients will be expected to make their own way to the surgery. The practice is not responsible to arrange transport for patients to travel to the surgery.
  • adults with common problems, such as a cough, sore throat, influenza, back pain and abdominal pain are able to come to the surgery.
  • common problems in the elderly, such as poor mobility, joint pain and general malaise will also be seen by appointment at the surgery.

 

Need a little help with the Evergreen App?

To help you get started with the Evergreen app and creating your account there are a series of help articles available.  Linking your account to your GP record will enable the full functionality of the app including records access and repeat prescriptions as well as enabling you to make requests for help online.

You can link using an NHS login which is the easiest method for both you and the practice team, or by using linkage codes supplied by the practice.
