Practice StaffPractice Manager Our Practice Manager, Cheryl Hart, oversees the day-to-day administration and smooth running of the practice. Practice Nurse team Our practice nurse team and health care assistant see patients by appointment. They are involved in all the screening programmes and are competent at carrying out general health checks. Nurses are in surgery at various times; please request information at reception. There are also community (district) nurses. Community Nurses The Community Health Services employs district (community) nurses and health visitors etc: who are based at Five Elms Health Centre. They can be contacted by leaving a message at reception or by calling the Health Centre on 0208 276 7250. Health Visitor There is a health visitor who covers the practice.
